A 65-year-old woman presents with pain in her right shoulder, fatigue, and a productive cough. Her medical history is significant for type 2 diabetes and hypertension. Although she quit smoking 25 years ago, she has a 15-pack year history. A chest radiograph reveals a 5-cm left lower lobe mass with pleural effusion. A computed tomography (CT) scan confirms the findings of the radiograph and identifies several ipsilateral mediastinal nodes. Biopsy of several mediastinal nodes reveals a keratinizing squamous cell carcinoma. She received platinum doublet treatment in the first-line setting and showed a partial response for 8 months.
